Create dispute evidence

ENDPOINT/v1/disputes/{dispute_id}/evidence

Creates an evidence record on a dispute in NEW or PENDING_CUSTOMER and returns a presigned upload_url plus the evidence id. PUT the file bytes to upload_url before upload_url_expires_at. Send Idempotency-Key so retries return the original record.
