Create a monitoring-case file

ENDPOINT/v1/monitoring/cases/{case_id}/files

Creates a file record on a case that is not CLOSED and returns a presigned upload_url plus the file id. PUT the file bytes to upload_url before upload_url_expires_at. Send Idempotency-Key so retries return the original record.
